Palliative Care ACT (PCACT) is calling on Residential Aged Care Facilities across the ACT to ensure Advance Care Planning (ACP) documentation is uploaded to MyDHR.

PCACT has recently become aware of a number of instances where people have been transferred from residential aged care to hospital or Clare Holland House without their ACP documentation being readily available. As a result, healthcare professionals did not have timely access to the person’s wishes and preferences for their care when it was most needed.

For many people, advance care planning reflects careful thought and important conversations with family, loved ones and healthcare professionals. In 2026, a person’s wishes should not be left behind when they move between care settings.

PCACT strongly encourages all ACT Residential Aged Care Facilities to upload residents’ ACP documentation to MyDHR, unless a resident chooses otherwise. We also recognise that some facilities may face genuine barriers to uploading these documents. If that is the case, PCACT would like to hear from you so we can better understand the challenges and work together to find practical solutions.
